II.2.4) Description of the procurement
The purpose of this tender is to commission housing related support within Ty Annog, a 24 hour supported housing scheme for young people aged 16-24 years old from Blaenau Gwent. Resettlement support will also be provided to the young people who move on (from Ty Annog) into their own accommodation in the community.
The services will be provided by one Support Provider in order to provide clarity and continuity between the housing support provision and the resettlement support service.
