II.1.4) Short description
Conwy in collaboration with Denbighshire County Council wishes to appoint a suitable, qualified and experienced Service Provider to deliver Shared Lives Services across the counties of Conwy and Denbighshire applicable for individuals with a varied range of needs. The successful Service Provider will be required to work in partnership with both Councils to further develop the Shared Lives Service across the two counties and promote independent living. The Contract awarded will be for a duration of seven years with an option to extend further for a three year period. Intentions are to commence the contract immediately in the new financial year 2020/21 with a date to be further agreed, subject to successful completion of the tender exercise.

II.2.4) Description of the procurement
The procurement relates to the provision of Shared Lives Services offering a valuable alternative and highly flexible form of accommodation in the local community and/or care or support within and outside of the Shared Lives Carer’s home. The Shared Lives Service includes provision of long term (previously known as adult placements) and short term placements (short breaks)and day time support in the Shared Lives Carers' home. Conwy County Borough Council holds a current contract for a Shared Lives service with an independent organisation which expires on the 31st March 2020. Within Denbighshire, this Service is currently delivered by in-house services and is planned to be externalised under the joint contract with Conwy.
