Mae’n rhaid i’r Darparwr Gwasanaeth fod â’r arbenigedd sy’n angenrheidiol i ddarparu gofal rhagweithiol a chreadigol sy’n canolbwyntio ar yr unigolion er mwyn iddynt allu byw bywydau iach a diogel. Bydd y contract yn cael ei roi am dair blynedd gydag estyniad o hyd at ddwy flynedd. Mae angen i’r gwasanaeth ddechrau ar unwaith ar ôl dyfarnu’r contract yn unol â’r terfynau amser sydd wedi’u nodi yng nghynnig tendr y Darparwr Gwasanaeth.

The Service Provider must have the necessary expertise and specialism in order to provide proactive, creative, person centred care to enable them to live active, healthy and safe lives. The Contract awarded will be for the duration of three years with an extension for up to two years. The service is required to commence immediately following Contract Award in accordance with the timescales specified in the Service Provider’s tender proposal.
