Merthyr Tydfil County Borough Council is seeking tenders from reputable and qualified providers for leaving care services.
The Leaving Care Team will support young people who are leaving care, as defined by Social Services & Well-being (Wales) Act 2014 and Part 6 Code of Practice; enabling them to transition into independence.

II.2.4) Description of the procurement
The objectives of the service are
1.To provide a Young Persons Advisor Service to young people leaving the care service aged 16 to 25 years
2.In partnership with Children’s services, conduct a programme of Information and advice workshops supporting areas of the pathway plan for young people in care aged 16 to 18 years of age
3.To develop and review high quality, person centred outcome focused Pathway Plans for each young person that underpin stable lives and successful outcomes
4.To develop at least 3 Supported Lodgings Placements that are appropriate for young people aged 16 plus
5.To contribute to management information and support to the permanence team for future planning of service provision
6.To work in partnership with stakeholders in developing opportunities for care leavers in line with the Council’s offer under Hidden Ambitions
7.Involving care leavers in the development, delivery and improvement of the services lies at the heart of the delivery which should have a strong focus upon co-production
In accordance with Part 6 (Looked After & Accommodated Children) of the Social Services & Well-being (Wales) Act 2014; Leaving Care. This incorporates the arrangements for leaving care, personal advisors, pathway assessments and plans, suitable accommodation and support for further or higher education and employment.
