II.2.4) Description of the procurement
Bridgend County Borough Council’s Social Services and Wellbeing Directorate wishes to commission a specialist supported living service located in Cwmfelin, Maesteg for up to four Individuals with a learning disability and complex challenging behaviour, including autistic spectrum conditions. We would be interested in bids from experienced organisations, with a proven track record in successfully delivering a high specification service of this kind.
The contract, which is for four years with the option to extend for a further 12 months, will commence on 1st November 2019 and it is an expectation of the commissioned Service, that the Provider will work with Individuals, their Care Co-ordinators and relevant Multi-Disciplinary Teams, including Health Professionals to deliver a customised package of Care and Support for each Individual and to ensure that they are supported to make their own decisions, take control of their lives, experience the same range of opportunities and life experiences as other people in the community and maintain independence using a model of Active Support.
