Lot 2 - Provision of an Integrated Housing Support Service using the Principles of Housing First
The Contract will be for a 4 year period with an option to extend for a further period of up to 36 months, subject to available budget, satisfactory performance and agreement by both parties.
It has been confirmed by the current provider of this service that there are TUPE (Transfer of Undertakings (Protection of Employment) Regulations 2006) implications attached to this contract.

II.2.4) Description of the procurement
Model - To provide an integrated housing support service for people aged 25 and over who are homeless or at risk of homelessness and in need of intensive, housing support.
Access Referrals are to be made via Carmarthenshire County Council Housing Options team.
Accommodation - Accommodation will be secured separate to this service.
Location - The service will be provided in the Ammanford, Carmarthen and Llanelli areas of the county.
Client Group - People who are leaving prison with co-occurring needs; People who are / or at risk of homelessness and who have a history of offending, mental health or substance misuse issues.
Length of Support - There is no time limit to this service. The length of support will be determined by the needs of the person being supported as detailed in the person’s plan of support.
The supported accommodation units will be available for people who require 24hour staff presence in a more structured environment and can be used to assist people resettle back into the community. As such it is expected the need for this type of housing solution will be of shorter duration, normally up to 6 months.
Hours of support will be split across -
Carmarthen / Ammanford Area:
4 units of 24/7 supported accommodation
4 units of move-on accommodation in close proximity to the supported accommodation units
5 units of shared community housing
Llanelli Area:
2 units of move-on accommodation
18 units of shared community housing
Multi-agency working - Providers will work with statutory services and other local agencies in Carmarthenshire to deliver a co-ordinated, joined up package of support.
This service is aimed at people who are aged 25 and over, who require support to maintain their accommodation and their home in one of the accommodation units detailed above. The Service Provider will be responsible for the support provided in these units.

II.2.4) Description of the procurement
1.1 Model - To provide an integrated housing support service for people aged 18 and over who are homeless or at risk of homelessness and in need of intensive, housing support.
1.2 Access - Referrals are to be made via Carmarthenshire County Council Housing Options team.
1.3 Accommodation - Support is not tied to any particular accommodation. Therefore, this service is a non-accommodation based service.
1.4 Location - The service will be provided across Carmarthenshire.
1.5 Client Group - People who are leaving prison with co-existing needs; People who are, or at risk of being homeless and have a history of offending, mental health or substance misuse issues.
1.6 Length of Support - There is no time limit to this service. The length of support will be determined by the needs of the person being supported as detailed in their plan of support.
1.7 Number of Hours - 172 hours of support expected to be delivered over 12 units.
This service will provide an intensive package of floating support to people who are aged 18 and over and who are homeless, at risk of homelessness and who have multiple, co-occurring needs. The floating support will include supporting the person find suitable accommodation.
The client group to be supported includes prison leavers who are at risk of homelessness. Supporting the Welsh Government’s prisoner pathway, this service will work with people leaving prison to source and secure permanent accommodation and support them to re-establish themselves in the community.
