Camden CCG’s Community Chronic Kidney disease (CKD) and hypertension services are currently provided by Royal Free London Hospital in separate contracts. A recent review envisaged that an integrated CKD and hypertension service will support and align future long-term conditions for patients. The proposal is for an integrated CKD and hypertension service with RFL for a period of 21 months from 1-7-18.

II.2.4) Description of the procurement
RFL currently delivers separate community based services for CKD and hypertension patients. Current contracts for these services were terminated on 30-6-18 to form a new integrated service (commencing 1-7-18), benefitting patients with co-morbidities and releasing efficiencies for the CCG. Developing and mobilising an alternative form of services provision would pose a serious challenge and may not be feasible for an alternative provider. Camden CCG will gain consistency, quality and better outcomes from RFL with this new arrangement. The long term plan for Camden CCG is to provide an integrated LTCs service for the local population from 2021/22 and opportunities will be available to express an interest in developing this wider integrated service later in 2020

IV.1) Description
IV.1.1) Type of procedure

Justification for selected award procedure:
The procurement falls outside the scope of application of the Directive
Explanation:
Chronic Kidney Disease (CKD) and Hypertension services were previously separately commissioned with the Royal Free London NHS Foundation Trust (RFL). Both of those contracts were due to expire on 30.6.2018. Following a review, Camden CCG assessed the different options available to continue both the services, achieving synergy and economies of scale. This review concluded that the preferred way forward was to recommission the CKD and Hypertension services as an integrated service which would be commissioned until March 2020. This would provide time to both consolidate these two services as well as allow further time to explore wider integration with other specialist community services to support those with chronic and/or long term conditions. In addition, due to the close relationship between CKD and Hypertension will create additional Quality, Innovation, Productivity and Prevention (QIPP) improvements, such as better self-care, prevention as well as financial efficiencies.
