II.2.4) Description of the procurement
Provision of a recovery based model of outreach support (Care at Home and Housing Support) for adults (aged 16 plus) with mental health difficulties, mental illness and personality disorder.
The service shall deliver 340 hours of direct support per week.
The contract award will be for a period of 3 years, with 2 1-year extensions at the discretion of the council.

Bidders will be required to provide examples that demonstrate that they have the relevant experience to deliver the services as described. A minimum of 2 examples will be required.
