Abstract

Wrexham County Borough Council and Betsi Cadwaladr University Health Board are in receipt of Welsh Government Regional Integration Fund (RIF) funding and are looking for applications from third sector providers: The purpose of the Service is to provide a Community Agents Scheme for people residing within agreed areas of Wrexham County Borough who meet the service criteria. Area’s: Abenbury Community Council, Bronington Community Council, Erbistock Community Council, Hanmer Community Council, Holt Community Council, Isycoed Community Council, Maelor South Community Council, Marchwiel Community Council, Sesswick Community Council, Willington, Worthenbury Community Council Esclusham Community Council, Llangollen Rural Community Council, Chirk Town Council, Ruabon Community Council and Overton Community Council. The successful Provider will work with and support individuals and communities in the above areas of Wrexham County Borough, through the provision of a Community Agent and will also promote the uptake of the community inclusion grant applications which provide seed funding to community and third sector organisations to establish community activities and services for older people, the aim being to reduce social isolation and loneliness and improve health & wellbeing, enabling older people to live within their homes and communities for longer The contract will provide Community Agents to help older people find their way to activities or services which they would enjoy or find useful. Making the most of local activities and services is a good way to keep fit, active and independent, but not everyone knows what is available. People with health or other difficulties may need that bit of help to access and benefit from activities and services available to everyone. This should be achieved through working in a person centred way and ensuring that the focus is on re-ablement / enablement, recognising individuals skills; knowledge and abilities. Agents will actively build relationships with statutory, community, third sector and independent organisations i.e. Police, Fire Service, Ambulance Service, GPs, visiting wardens, local libraries & leisure facilities and housing, promoting access and inclusion. The aims and objectives of the scheme • To support older people to remain active and engaged within their communities • To prevent early hospital admission and assist with early discharge through building supportive and age-friendly communities • Community Agents will provide information, advice and support on local community activities, services and sources of support • Support for local organisations to develop services accessing the Council’s community inclusion grant and external funding. • To promote and support social, health and well-being with a focus on peoples abilities, knowledge and skills through a re-ablement / enablement approach. The Provider will ensure that all aspects of their service are delivered within the principles of: A Strategy for an ageing Society 2022 – Creating Age Friendly Communities in Wales. Sustainable Social Services for Wales: A Framework for Action and The Health & Social Care Well Being Bill which promotes “preventative” services being available to people who are currently eligible to receive social care services but also to the wider population of those whom are on the periphery of services. The contract will commence on 1st April 2024 unless already specified above, and will expire on 31 March 2025, with option to extend. The contract will be reviewed annually and will be subject to a 3 month termination clause. TUPE will be applicable as there is an incumbent provider delivering the service at present. To register an application, please submit a written statement of the offer you are able to make.
