Conwy County Borough Council wishes to appoint a suitable, qualified and experienced Domiciliary Care Provider to deliver supported living services to three adult male Individuals who will be moving from their existing settings to live together in a shared Supported Living Scheme within Conwy. The tender will be let under the The Light Touch Regime of the Procurement Regulations 2015.

II.2.4) Description of the procurement
The Service required is to provide 24 hour person centred support to three Individuals with a varied range of needs including learning disabilities, autism, challenging behaviour and epilepsy. Due to the high support needs of the Individuals, the Service Provider must have the necessary expertise and specialism in order to provide proactive, creative, person centred care to enable them to live active, healthy and safe lives. The Individuals will be supported to live independently in the community in the Conwy area (Kinmel Bay). The Contract awarded will be for the duration of three years with an extension for a two year period. The service is required to commence immediately following Contract Award in accordance with the timescales specified in the Service Provider’s tender proposal.
