Abstract

CTMUHB Therapy services require up to 6 months of programme support for the delivery of Phase 1 of the Long term Conditions Rehabilitation Programme. Our new strategy, CTM 2030: Our Health Our Future, aims to unite CTM partners and stakeholders, patients / service users, all of social care and the third sector around a shared understanding of the everyday things affecting people’s (or those they care for) health and wellbeing in our region; what does good health really mean to our communities? There are important questions we need to ask and understand. Which services give the best value for our patients? What could we be doing differently to help people stay healthier longer in their own homes and communities? How do we improve quality and outcomes for our communities?  How do we make best use of our buildings and community assets?  How do we develop healthcare services in a ‘green way’?
