HDD-ITT-47586 - Provision of Mental Health Support Services – Amman Gwendraeth

Description of the goods or services required
Hywel Dda University Health Board are seeking to commission mental health support services for adults (18+ years) with mild to moderate mental health concerns to support to all eight GP Practices within the Amman Gwendraeth Cluster. The cluster are looking to develop a community based service to provide patients with the support needed to enhance their mental well-being by offering a personal approach outside of counselling, for an initial period until 31st March 2022 with an option to extend for a further 12 months should funding continue to be available.
The service will offer a combination of face to face and virtual sessions and aim to deliver an efficient service across Amman Gwendraeth. Based on historical data the service should be prepared to accommodate in the region of 50 referrals per week, these volumes are not guaranteed and are subject to change based on demand.
