The aim of the service is to enable people to ‘live the best life they want to live’ in an environment where the service is the enabler, and the people are the changers. Supporting independence within the person’s home – to increase the range of options for self-support and targeted assistance to build on their strengths and maintain or improve their quality of life.
A fundamental principle is an emphasis on voice, choice, and control to maximise outcomes. The successful provider must share this vision and work in partnership with the Registered Social Landlord (RSL) and the local authority, to connect and integrate individuals within their local community to access a range of facilities. The successful provider must enable individuals to maintain an appropriate level of independence with the appropriate level of care and support as and when needed.
Extra Care takes into consideration specialist requirements for people who may be presenting with other care and support needs including cognitive decline and sensory impairment.
