II.2.4) Description of the procurement
Description of Service
-To educate and empower the pupils, their parents, and teachers about asthma and to enable them to self-manage their condition.
-Conduct the reviews in a child friendly environment The service is to be run 4 to 5 days per week during school working hours, for individuals residing within the Pembrokeshire locality and registered at a GP practice within Pembrokeshire.
-During school’s holidays training sessions are to be provided to support individuals within such groups as, summer clubs and sports and recreational clubs, in the management of asthma in primary school children. The provider will be expected to link in with community services and sports groups within the Pembrokeshire locality
-The service provider will also need link in with other stakeholders such as Smoking Cessation Services and Secondary Care Paediatricians.
