Abstract

Expression of Interest - Provision of Long Term Placements (shared lives) for adults with disabilities and older people WCBC - Wrexham Adult Social Care Department wishes to commission an Adult Placement Service for older people, (age 50+), people with a learning or physical disability, as well as for people with mental health needs, on a spot contracted basis. The Shared Lives Scheme (formerly Adult Placement) shall comprise the provision of accommodation and care and support within the home of Shared Lives Carers in the community. The Scheme provides Service Users with an option of living within a family environment in the community. The delivery of Long Term Placements must be in accordance with the requirements in the Social Services and Well-being (Wales) Act 2014 and the associated Code of Practice. We would be interested to understand what services are available, and an understanding of the costs associated with the delivery of the service. The Local Authority is posting this notice to ascertain the level of interest in the market to provide this service with a view to a full tender exercise.
