II.2.4) Description of the procurement
The purpose of this tender is to commission a Service Provider to deliver a high quality 24 hour outcome focussed care and support service within a supported housing environment to 5 individuals with learning disabilities who also display behaviour that challenges, have co-morbid conditions, have additional physical/sensory disabilities and/or present as having additional complex needs. All 5 individuals have high care and support needs. Lamb House, currently owned by Aneurin Bevan University Health Board, is a 5 bedroomed supported housing scheme and is located in the town of Blaina within the borough of Blaenau Gwent. The care and support provision will enable the 5 individuals to have the opportunities to live as independently as possible so they can enjoy a fulfilling life.

This tender will run for four years commencing 1st April 2019 with an option to extend for a further 24 months
