Abstract

Are you a Third Sector organisation with experience of training, developing and embedding person centred approaches with people with learning disabilities, families and Providers? The North Wales Together Learning Disabilities Transformation team are working with Conwy, Denbighshire and Flintshire Learning Disability teams to help support and promote person centred practice across both counties. There is a maximum budget of £7,000 available for a 4-month period to fund a programme of PCP citizen, family and provider engagement and to draft a Report The third sector organisation will be expected to engage with citizens, their families and local providers, so as to get their feedback on how best to improve person centred practice. The engagement work will need to include:  Scope the current approaches to PCP and review PCP templates and tools being used (so as to get feedback from the perspective of individuals and their families). Facilitate small group sessions / workshops with families, citizens and providers:  Engaging with young people and their families to help gauge their experiences of PCP in schools and to understand whether and how PC action plans are carried forward into adulthood.  Finding out about peoples understanding and experiences of PCP.  Discussing and identifying what needs to happen to improve people’s experiences of PCP and co-producing their recommendations about they’d like to see, moving forward.  Setting out the findings from this work so as to help shape and inform future PCP practice. We will require a Report that will be used by the North Wales Together Learning Disability Programme to form a Strategy around the promotion of PCP across the region. Similar pieces of work will be carried out across each LA area from the Service perspective. The successful provider will need to demonstrate a co-productive approach with families and citizens to the delivery of proposed training. They will be required to create opportunities for family involvement as critical partners in the planning process. If you have experience of engaging with people with learning disabilities and of developing and embedding person centred approaches with people with citizens, families and staff and you would like to submit an expression of interest, please contact us at learning.disability.transformation@flintshire.gov.uk Please provide us with the following information by the 25th of November 2022: a. Tell us what experience your organisation has got of engaging with individuals with learning disabilities and their families, what the challenges are, and how these can be overcome. b. Tell us about your organisations experience of PCP c. Provide us with a work-plan (a broad outline of what you will do, how you will do it and associated costings) Please use no more than 500 words per answer
