Abstract

Cardiff City and South Cluster encompasses 6 GP practices, with a combined registered population of circa 41,000. Through use of Welsh Government Funding, this cluster is seeking to establish a social prescribing project focused on the use of gardening to improve the health and wellbeing of patients in their local area. Through use of a social prescribing approach, the practices feel that their patients will experience the following benefits: • Social interaction - reducing the impact of social isolation. • Sense of purpose - improving levels of self-resilience, confidence and independence • Exercise - improving general levels of mental and physical health. In terms of benefits to the GP Practices, it is believed that access to this scheme could provide a more appropriate intervention for patients who do not require clinical intervention via GP Practice.

Key Measures of Success:

Having trialled the introduction of this model previously, the commissioner is seeking:

• Demonstrable evidence that all practices have been encouraged to engage in the project.

• Evidence that at least 4 of the 6 practices have referred patients to the project during the project.

• To enroll a minimum of 50 new patients into the scheme over the course of 18 months, with evidence of patients completing a course of intervention- NB: It is accepted that the level of referrals into the service is dependent upon engagement from practices and this will be monitored throughout the course of the project.

• The Commissioner is also keen to create referral pathways from other health professionals/cluster based services where appropriate to maximise use of the service

• To provide evidence of positive patient outcomes i.e. improved levels of personal wellbeing based on ‘what Matters to You’ principles

• To demonstrate positive feedback from the practices as to the benefits of this model for their patients with case stories/data which indicates that reliance on GP care may have reduced as a consequence of social prescribing.

3 Conditions for Participation

3.1

Minimum standards and qualification required

The provider will be required to:

• Work with the cluster to design, project manage the set-up and co-ordinate the ongoing day-to-day site management of the schemes within the identified areas.

• To recruit and manage any staff that are needed within the project (ie all employer liabilities will sit with the provider), this includes coverage of any staffing gaps outside of usual annual leave allowances.

• Bidders are expected to identify and cost the equipment required to provide the project. Once purchased, the Provider will need to ensure safe storage of the equipment and confirm return of equipment to the UHB should the project be discontinued.

• To work with the cluster to confirm the deliverable and outcomes for the project and produce reports on a frequency specified by the Cluster.

• To attend regular contract monitoring meetings as requested

• Ensure that access to gardening should be as inclusive as possible. The cluster is keen to attract patients from a wide breadth of ethnic groups as well as users of different abilities.
