Health and social work services. Health services. Miscellaneous health services. A holistic service to patients with mental health needs across 37 GP Practices (which includes; psychological intervention; social prescribing; care coordination; and education and training).

Team around the Practice (TAP) will be a psychiatrist-led and multi-disciplinary team within primary care to provide holistic support to people with mental health problems. The service will coordinate access to a wide range of services across health, social care and third sector.
