They support individuals to maximise their potential and remain as independent as possible whilst avoiding long stays in hospital. Staff within the extra care scheme work alongside the integrated health and social care team to ensure patients’ care and support needs are met and this contract ensures that this is in place for the step up/step down bed in Thirsk.

Explanation
During 2016/17 the CCG approved the commissioning of a number of step up/step down beds by utilising extra supported housing and care schemes. It was agreed that these would be developed incrementally and in a phased approach in order to prove the concept in a number of different locations and providing a service for patients with differing needs.
Phase 1 commencement of implementation saw the commissioning of 1 bed at the extra care housing schemes in Bainbridge as the pilot project.
Phase 2 included the commissioning of a further 7 beds in Thirsk, Stokesley, Leyburn and Bedale.
Phase 3 extended the service to the Northallerton Locality with an initial allocation of 1 bed.
The majority of beds have been commissioned through the Section 256 agreement with North Yorkshire County Council (NYCC) on the basis that the council already hold contracts with a number of extra supported housing and care schemes across the CCGs area. However NYCC do not hold a contract with the extra care housing scheme at Orchid House, Acadia Drive, Sowerby, Thirsk, YO7 3QA. In response to the recommendations of the “Transforming our community” public consultation and approval by the governing body in October 2016 of the step up/step down implementation plan the CCG were required to commission beds within the Thirsk locality area. At this time Orchid House is the only extra care housing facility in this area and Thirteen Housing Group is the only (care) provider available in this area. When benchmarking the rates quoted against the other beds currently commissioned through the Section 256 the CCG is assured that the provider is offering value for money.
