b) Outreach- Offering 1:1 (or staffing ration in line with assessed need) support to children and young people within the community (or home setting if required) to work towards outcomes identified within the support plan.
Where shared outreach services (2:1 child: staff ratio, or other suitable ratios) would be beneficial to children and young people these may be agreed through Stockton on Tees Borough Council's panel process, subject to risk assessment and agreement with the council.
