Needs analysis, service gap identification, and market engagement have identified that for DCCS to deliver an improved, integrated and improved outcomes — based early intervention/preventative offer to adults the department should put in place a coordinated supplier infrastructure which will move the department towards developing a system approach with consistent and interlinked outcomes which will further promote independence, delay or reduce the development of health and social care needs and reduce hospital admissions.
