Mae Cyngor Bwrdeistref Sirol Conwy yn dymuno comisiynu sefydliad cymwys, addas i gydlynu a darparu Gwasanaeth Cydlynu Gwirfoddolwyr (Cefnogi Teuluoedd) fel rhan o’r grant Plant a Chymunedau – rhaglenni Teuluoedd yn Gyntaf a Dechrau'n Deg.
Mae’r rhaglenni Teuluoedd yn Gyntaf a Dechrau'n Deg yn rhan o grant Plant a Chymunedau Llywodraeth Cymru, sydd â’r nod o fynd i’r afael ag anghenion cefnogaeth y plant ac oedolion mwyaf diamddiffyn yn ein cymunedau, drwy ystod o fecanweithiau ymyrraeth gynnar, atal a chefnogi. Bydd yn ceisio lleihau neu ddileu'r anfantais i bobl ddiamddiffyn, er mwyn eu galluogi i gael yr un cyfleoedd mewn bywyd ag eraill, ac felly cyfrannu tuag at Gymru sy’n fwy cyfartal.
Bydd angen i'r Gwasanaeth sy’n cael y Gwahoddiad i Dendro weithio gyda theuluoedd ledled Conwy a gyfeirir gan Dimau Cefnogi Teuluoedd Conwy i gael Cefnogaeth un i un gan wirfoddolwyr yn ogystal â chefnogi grwpiau a gyflwynir gan Dimau Cefnogi Teuluoedd. Nod y gwasanaeth fydd gweithio o fewn y model canlynol ar gyfer Cefnogi Teuluoedd yng Nghonwy;

Lefel 2: Cefnogaeth un i un i deuluoedd gan wirfoddolwyr
Lefel 3: Cefnogaeth un i un ddwys i deuluoedd gan Weithwyr Teulu
Mae gofynion manwl ar gyfer y Gwasanaeth Cydlynu Gwirfoddolwyr (Cefnogi Teuluoedd) wedi’u nodi yn y Gwahoddiad i Dendro llawn, a’r dogfennau Manyleb Dendro a Chyfarwyddiadau i Gynigwyr.
Conwy County Borough Council wishes to commission a suitably qualified organisation to co-ordinate and deliver a Volunteer Co-ordination (Family Support) Service as part of the Children and Communities grant – Families First and Flying Start programmes.
The Families First and Flying Start programmes are part of the Welsh Government’s Children and Communities grant (CCG) which aims to address the support needs of the most vulnerable children and adults in our communities through a range of early intervention, prevention and support mechanisms. It will seek to mitigate or remove disadvantage to vulnerable people to enable them to have the same life chances as others, and therefore contribute to a more equal Wales.
The Service covered by this Invitation to Tender is required to work with family units across Conwy who are referred by Conwy Family Support Teams to receive one to one Family Support from volunteers as well as supporting groups delivered by Family Support Teams. The Service will aim to work within the following model for Family Support in Conwy;
Level 1: Unstructured and structured groups supported by volunteers
Level 2: One to one family support by volunteers
Level 3: Intensive one to one family support by Family Workers
