The Sheffield Dementia Strategy (2019-2024) was developed together by people living with dementia, their families, and the organisations working to support them. The strategy aims to make sure that people living with dementia of all ages and their families are supported to live to their full potential. It includes 13 commitments which describe how we will do this in Sheffield. Its development was linked to the Prime Minister’s 2020 Challenge on Dementia.

II.2.4) Description of the procurement
The service will contribute towards improving the care we provide to people with dementia. It will support people living with dementia who are starting to struggle with their situation and present at Primary Care or other contact point. It will aim to prevent further deterioration and potential crisis. <br/>• Outcomes delivered by the service for people living with dementia will be: <br/>• Increased resilience<br/>• Helping people to live well in their community<br/>• Improving knowledge and understanding of living with dementia <br/>• Preventing social isolation<br/>• Maintaining independence, choice and control<br/>• Accessing and building wide support networks <br/>• Improving coping strategies and recognising increasing difficulties <br/>• Improving access to diagnosis of dementia.
