Abstract

This Contract is for the provision of a Step-Down service in Oxfordshire. The provider is to deliver both the accommodation and staff. Step-Down housing is classified as Discharge to Assess (D2A) Pathway 2 intermediate care. It is short-term accommodation (typically up to six weeks) for people who are ready to be discharged from hospital but who are either homeless, threatened with homelessness or whose housing is not suitable to meet their needs (for example it may require major adaptations or a deep clean). A stay in Step-Down is a chance to ‘catch your breath’ whilst your ongoing care, support and housing needs are assessed and measures put in place that aid recuperation, maximise the potential for positive outcomes and reduce the chance of readmission to hospital or rough sleeping and homelessness.
