Combined community tissue viability service and lymphedema service. This combination will ensure that all patients at risk of or with tissue viability conditions receive effective diagnosis and targeted self-management support, monitoring, treatment and care in order to maintain the highest quality of life and independence possible.

II.2.4) Description of the procurement
Combined Lymphoedema and Tissue Viability services. This is a light touch regime procurement. The total value as stated within this notice is GBP 1 357 100. The annual estimated value is GBP 271 420 per year. A 3 year contract will therefore have a value of GBP 814 260. There will be an option (at the CCG’s sole discretion) to extend the contract for a period of up to 2 further years thereafter. A 3 year contract plus an extension of a further 2 years thereafter would have an estimated value of GBP 1 357 100. The duration has therefore been shown as 60 months.
