Abstract

Short term assessment facilities are a developing model of care which maximises the potential for recuperation and recovery and optimise holistic well-being, with the shared reablement ethos of providing the right care; at the right time; in the right place. They also facilitate timely hospital discharges and help to avoid unnecessary hospital admission.

Through this framework the Council will commission beds within residential care homes for adults, specifically to enable people who need a short period of care away from their own home, to have the best opportunity of returning home.

As the delivery of personal care is included in the specification, care homes must be appropriately registered with the regulatory body, the care quality commission.

Inclusion on the framework does not guarantee the commissioning of any work with a provider.

The Framework Agreement will be established for an initial term of 2 years, with an option to extend for a further period of up to 2 years.
