Abstract

The service promotes a pathway to future independent living, greater social inclusion, through location of people within a community. The service offers a safe environment with the support of on-site staff through-out the day and the security of overnight staff. The care and support are based on individual need which may include the use of assistive technology where appropriate.

The accommodation provides care and support for up to seven people at full capacity.

This comprises of:

• 7 x self-contained flats, each containing 1 bedroom, bathroom and kitchen diner with a garden area. The 7 people will represent the core cohort and form the basis of the service

• A self-contained ground floor one bedroom flat, for the successful Provider to operate from.

The seven people living in one of the self-contained flats do so under a license agreement (limited to two years).

The Council manages referrals for the self-contained flats, in all cases they will be for short term use.

The 7 flats are commissioned by using Individual Service Funds (ISF) which can be used flexibly to support people to achieve their outcomes.

The service supports adults who are eligible for social care support as a result of being vulnerable or their lifestyle. The age range of people to be accommodated and referred within the service is 17.5 years to 28 years.

Adults will be resident within Newcastle upon Tyne and be assessed as eligible to receive care under Care Act 2014, following a needs assessment.
