Description of the goods or services required
Bridgend County Borough Council is commissioning a Dementia, Memory Impairment and Emotional Wellbeing Support Service for Individuals who have a cognitive impairment within Bridgend County Borough. The Service will support Individuals who experience cognitive impairment, and who may also have other needs, to acquire skills, build resilience and maintain independence.
The Council wishes to appoint one Provider that has extensive knowledge and proven experience in providing person centred support, developing strategies and techniques to support independent living, and provision of information and signposting to support Individuals with dementia and memory impairment, and their Carers. The Provider will have a comprehensive knowledge of relevant services and service pathways, groups, facilities, and activities within Bridgend County Borough that would be beneficial to Individuals, and/or their Carers, who utilise the Service, and in addition provide information on relevant schemes and benefits that may offer support to Individuals and/or their Carers. In addition, the successful Provider will also have adequately demonstrated they have the skills, experience and understanding of undertaking this work in a social care environment, and in particular to people Individuals who may be regarded as vulnerable and with a range of support needs.
The Provider’s Staff will work as part of the Complex Integrated Day Service; the Community Integrated Teams and the Older Persons Mental Health Team in achieving the objectives of the Service, and the Provider’s Staff will be co-located with these teams in Bridgend.
Contract term: 10 Calendar Months from 1st June 2024, with the option to extend up to a further 12 months subject to the Regional Integration Fund.
