Tunbridge Wells Borough Council (“the Council”) is conducting a tender exercise with and
on behalf of its partner authorities, Sevenoaks District Council and Tonbridge and Malling
Borough Council, to award a number of contracts for the provision of Hospital Discharge,
Home Straight and Handyperson services. These include:

Lot 1. Hospital Discharge Service – All partners
The purpose of the service is to provide practical support to help patients with a housing related issue to leave hospital and go home in a safe and timely manner, and to deliver interventions to prevent hospital admission or readmission. The service works closely with NHS colleagues within the hospital discharge teams at Maidstone and Tunbridge Wells NHS Trust.
The service is open to anyone who is being discharged from hospital and has a housing related issue, or who is at risk of an unplanned admission or re-admission, where there is an unresolved housing issue affecting their wellbeing or has the potential to cause a stay in hospital.
