Transitional and Reablement Beds will provide various levels of support to Older Persons who require a period of further assessment to determine their longer-term needs with the focus being on returning home. This will be achieved in partnership between Purchaser and the Provider.
The service will provide early intervention, assessment, support and care planning for the Service User and will work with the individual and the Care Home staff to help achieve their identified goals and outcomes.
The procurement will be carried out by Lincolnshire County Council (LCC) on behalf of Lincolnshire Community Health Services (LCHS). The requirement is for a block of fifty (50) Transitional Care beds, twenty seven (27) of which are Nursing beds with the remaining twenty three (23) being residential care beds.
