Abstract

The service will aim to assist with the timely discharge of patients from acute hospitals to residential care homes. ​

The trusted assessor will seek to ​:

— reduce time from patient becoming medically fit for discharge to arriving at care homes; (Links to high impact change model for — managing transfers of care – HICHM 1),

— improve communication between hospital and care homes to aid timely discharge,

— assist care home managers with assessment process (Links to HICM 3 and 6 – multi disciplinary, multi agency discharge teams and trusted assessors),

— provide liaison where there is a potential change of needs,

— reduce the need for care home manager to visit hospital,

— if possible leverage working together on this project to encourage wider working together between independent sector providers.

The authority is issuing this invitation to tender (‘ITT’) in connection with the procurement described in the advertisement placed in the Official Journal of the European Union with reference number 2019/S 135-332421.

The procurement is being run under the open procedure, which is a formal bidding procedure, under which the contract is advertised and all interested organisations/consortium can bid. This ITT provides further details of the proposal and process for submitting bids.

This procurement is for the provision of early intervention and prevention services to enable people to be effectively and efficiently discharged from hospital and positively impact DTOCs as well as admissions avoidance initiatives and to respond to demand peaks in the health and care system, e.g. winter pressures.

The service will aim to assist with support at home for older adults who have been discharged from hospital, with a view to re-establishing their independence.

The support at home services will seek to:

— provide free support to people who have been unwell (aged 60+) to continue to live independently at home by providing support and contact (Monday to Friday), either by a personal visit or telephone call, to provide a sense of security and well-being,

— provide longer-term support depending on the service user’s need, assessed by professionals,

— contribute to the prevention of unnecessary hospital re-admissions by enabling individuals to remain safe and independent at home,

— improve signposting to statutory and voluntary agencies,

— support frequent hospital attendees to access services more appropriate to need.

The sensory services will provide support for adults (i.e. those aged 18 and over) with sensory impairment, including profound hearing loss, who are resident in the local authority areas of Cambridgeshire and Peterborough. ​

The objective of this service is:​

— to maintain service user’s independence and stay living at home or within their family and prevent as far as possible the escalation of care or support needs. To prevent the occurrence of acquired sensory impairment,

— to apply the principles of: shared decision making with adults requiring support and their family or carers; interagency working and effective collaborations with clear communications, referral processes and data sharing that informs practice and service delivery,

— to improve, innovate and evidence the impact and effectiveness of this support service throughout the life of this contract,

— to develop and maintain the service provision within the [THRIVE framework]. This will ensure a needs based adult preventative service that is part of a whole system approach working toward improving the well-being, health, resilience and confidence of adults requiring support. ​

The Council would give any economic operator, once a year throughout the entire period of validity of the pseudo framework, the possibility of requesting to participate in the system and that the pseudo framework shall remain open throughout the period of validity of the framework to any economic operator which satisfies the selection criteria.
