Below provides a list of the services which are which are being put out to tender by NHS Oldham CCG

.

The lots are as follows:

- Lot 1 – Core Community Services

- Lot 2 – Specialist Elective Community Services

- Lot 3 – Continence Services

- Lot 4 – Intermediate Care Services

- Lot 5 – Respiratory Community Service

- Lot 6 – End of Life Community Service

Potential providers will be able to express an interest and complete a PQQ for any of the lots they wish to bid for. Bidders may apply for the lots they are interested in; this can be a combination of any of the lots on offer. It is anticipated that some bidders will bid for all lots; however each lot will be assessed and scored by the panel in isolation of each other.

Lot title: Core Community Services

All services within this lot must be delivered by one provider; submissions made for individual services will not be accepted. NHS Oldham CCG would like to offer the opportunity to deliver services under lot 1 & 2 to both large scale organisations wishing to tender for the entirety of the service, and also to smaller, more niche organisations wishing to tender for specific elements

Organisations wishing to supply the entirety of service would be expected to offer smaller organisations the opportunity to partner deliver (Lots 1 & 2) under a federated model. The CCG wishes to encourage innovative solutions, therefore those applying for lots 1 & 2 only, the requirement for partnership working to ensure fully integrated care will be an essential requirement

Potential providers are asked to consider the services included in this lot and ensure they can deliver each of these services before submitting a PQQ response.

The figures within the financial envelope are indicative 3 year figures based upon the services being commissioned within the specification.

Lot title: Specialist Elective Community Services
