Abstract

The development of a new reablement and intermediate care offer is a priority within the Rotherham Health and Social Care Place Plan. The NHS Long Term Plan and, more recently, national discharge guidelines introduced in response to the Covid Pandemic, outline an integrated ‘discharge to assess’ model based on ‘Home First’ principles. The aim is to develop community services to support more people to be discharged home directly from an acute setting or to remain at home during an exacerbation or acute incident. However, this is not always possible and there will be times when a person does not require consultant led care in an acute hospital, but cannot be safely cared for at home. There is therefore a need to provide community beds with nursing for short-term care. The required service will sit within the overarching Discharge to Assess Framework, supporting adults aged 18 years and over, providing step up and step down beds for those people whose needs cannot be met at home. Where appropriate, there will be a particular focus on recovery, reablement and rehabilitation to the level the individual can achieve. Suppliers will work with a multi-disciplinary health and social care team to asses an individual’s needs including their capacity for independent living and any potential ongoing care and support needs and the requirement will be to provide 24-hour nursing care beds support that focuses on reablement appropriate to each individual’s need.

The Authority is looking to work with a capable provider to supply a community unit with nursing to support individuals on a short-term basis who either cannot remain at home but do not require consultant led acute care or who are being discharged from acute care but are not yet able to return home. The supplier will work with health and social care providers to support discharge to assess, recovery, rehabilitation and reablement care plans to facilitate independence and enable as many people to return home as is possible.Individual therapy programmes will be formulated with the individual to achieve the following objectives :-• Return home at the earliest opportunity and/or;• Increase or maintain independence, choice and control and/or;• Instigate planned treatment and care to manage deterioration in health and wellbeing.
