Abstract

This service is linked to be delivered conjointly with an existing service contract which ends on 31.3.2020 but has the provision for an extension of up to 2 years to 31.3.2022.

Additional same day appointment capacity will be linked to the existing Merton Access Hubs service. This additional same day capacity would include face to face and video consultations.

The service would improve access to primary care services in Merton and support the direction of travel to enhance digital access to general practice in Merton and a ‘digital first’ approach. The capacity would be linked to the outputs of online symptom checkers and alternative digital/other routes through which a need for same day access is identified.

A digital first approach to patient access with an integrated front door to primary care services through an app, smart phone friendly websites, digital telephony and online symptom checkers. Patients will have the ability to book appointments online and have video consultations. There will be additional same day capacity to enable this and a joint commitment from all practices to provide dedicated time for multi-morbid patients ensuring overall access to primary care services for patients are safe and equitable.
