Abstract

Wrexham County Borough Council and Betsi Cadwaladr University Health Board are in receipt of Welsh Government Regional Integration Fund (RIF) funding and are looking for Expressions of Interest from registered residential care home providers, excluding those where the predominate client group has a dementia type illness, who are able to meet the required service aims and objectives as detailed below: Funding provided in this Contract is for the provision of up to 12 step up / down beds within residential care homes in Wrexham to complement existing county provision. The initiative forms part of Wrexham Adult Social Care and Betsi Cadwaladr University Health Board’s vision for Intermediate Care Services; and is jointly commissioned by the two partner organisations. The service aims to provide short-stay rehabilitative care within a residential setting, to support people coming out of hospital who are either medically fit but not yet ready to go home, where a domiciliary care package cannot be sourced in time to facilitate discharge, or to enable assessment to be undertaken outside of an acute setting. The beds are also to be used as a step up for people in the community to avoid hospital admission, long-term placement, as well as support crisis management. This service is for persons aged over 65 years, who reside in Wrexham County Borough, and whose needs can best be met through an Intermediate Care intervention rather than an acute hospital admission. Aims:  To provide short-term intensive rehabilitative step up / down support within a residential setting to enable people to return home from hospital sooner, overcome the need for a hospital admission in the first place, provide a place for assessment of long-term needs and/ or prevent premature admission to long-term care Objectives:  To provide timely access to short-stay beds  To work with Social Care and Health to ensure individuals placed are enabled to regain / maintain their independence in all aspects of daily living, in order to ensure a safe transfer back to their own home.  To support the implementation of ‘assess to admit’ and ‘discharge to assess’ pathways in Wrexham.  To provide high quality, outcomes-focused and dignified care to individuals placed within the beds  To maintain accurate records and produce regular monitoring information in line with the monitoring requirements of Health and Social Care. Providers will be expected to provide care, support and accommodation in line with their Statement of Purpose, CIW registration and the applicable Pre-Placement Agreement. Therapeutic interventions will be provided by the Wrexham Health and Social Care. Nursing input (where required) will be provided by either the District Nursing Team with responsibility for the home, or Health and Social Care, upon negotiation. For additional information please see the attached Service Specification. In addition to the above we are looking to seek interest in providing a planned respite provision with residential care homes to provide: 1 x Residential Care Respite bed – For immediate Commission 1x EMI Respite bed – For immediate Commission 1 x EMI step up/Step Down Bed – For Commission from 1st October 2022 The Respite beds will be for block purchase rotational respite of known citizens over the age of 65 under the care of Adult Social Care. The contract will commence on 1st October 2022 unless already specified above, and will expire on 31 March 2025. The contract will be reviewed annually and will be subject to a 3 month termination clause.
