Calderdale & Huddersfield Solutions Ltd (CHS) ("the Authority") is seeking a supplier for a Referral Triage, Advice and Guidance and Validation Service (Service) on behalf of Calderdale and Huddersfield NHS Foundation Trust (CHFT). Initially it is intended that the Service will be used by the Neurology Speciality (Neurology) within CHFT but potentially by other Divisions at a later date. Should there be a need for other divisions to use the Service then further details will be shared with the successful supplier.<br/><br/>The Service will cover:<br/><br/>1. New Referrals from GPs and Internally from Consultants- a triage service to determine whether patients are referred to the Trust or referred back to their GP.<br/>2. Advice and Guidance- to GPs relating to patients already being managed in the community which could result in a referral to the Trust.<br/>Validation Follow ups- patient case renewals to determine whether patients on a waiting list still need to be seen.<br/><br/>The Service will be required to be delivered remotely.
