II.1.4) Short description
The Tier 3 service across Norfolk and Waveney will offer the opportunity for service users who meet the qualifying criteria to access comprehensive support via a weight management programme tailored to their needs including reasonable adjustments in line with NICE Guidance. <br/><br/>The Tier 3 weight management service will incorporate both standard services and enhanced services for those service users with more complex health needs (co-morbidities) either independently or in partnership with other providers.<br/><br/>The service aims to:<br/><br/>• Encourage long term behaviour change by offering a programme which promotes physical activity; promotes healthy eating and supports the psychological barriers to unhealthy relationships with food; supports and recognises the relationship between mental health and obesity and offers a system throughout the programme which will support the psychological needs of everyone who attends.<br/>• Reduce the number of people living with obesity in Norfolk & Waveney, thereby improving long-term health, reducing the burden of disease, and decreasing the need for social care, while also preventing or reducing the incidence and healthcare activity related to obesity-related illnesses.<br/>• Function as part of a seamless care pathway for adults with obesity; to proactively work in partnership with services provided at tiers 1, 2 and 4, as well as other existing services supporting behaviour change<br/>• Undertake comprehensive stakeholder engagement to inform practice, referral confidence and workforce development.<br/>• Work with Norfolk & Waveney ICB using Population Health Management (PHM) data management systems to assist in the identification of service users with agreed BMI and clinical thresholds to ensure the people with the highest risk can be identified and supported to access help, where needed. This information can be produced at PCN / GP Practice level to risk stratify and prioritise patients, highlighting areas of highest need or clinical complexity.<br/><br/>The Contracting Authority will be using an eTendering system for this competitive process.
