Rydym yn chwilio am gyflenwyr i gyflawni contractau Cyflogaeth Lleoliad â Chymorth Unigol yn y sector cyflogadwyedd yn rhan o Raglen Cysylltu â Gwaith Gogledd Cymru, yn benodol yn Sir y Fflint. Bydd y contractau yn golygu cefnogi pobl ag anableddau sydd eisiau gweithio i ganfod cyflogaeth gynaliadwy yn lleol.

Mae Lleoliad â Chymorth Unigol yn integreiddio cefnogaeth â chyflogaeth ochr yn ochr â gwasanaethau iechyd sylfaenol ac eilaidd, a gwasanaethau cymorth eraill. Yn ddelfrydol bydd yr integreiddio yma’n digwydd ar lefel gorfforol, drwy gydleoli a pholisïau eraill, lle y bo’n briodol. Mae Lleoliad â Chymorth Unigol wedi cael ei ddefnyddio gan feysydd sy’n gweithio gyda’r Adran Gwaith a Phensiynau i gyflawni rhaglen ‘Lleoliad â Chymorth Unigol mewn Gofal Sylfaenol’ i gefnogi unigolion sydd â chyflyrau iechyd a/neu iechyd corfforol ysgafn i gymedrol. Bydd Cysylltu â Gwaith yn defnyddio ac yn ymestyn ar y dull a gymerwyd gyda Lleoliad â Chymorth Unigol mewn Gofal Sylfaenol. Er y bydd Cysylltu â Gwaith yn cyd-fynd â darpariaeth Lleoliad â Chymorth Unigol presennol ar gyfer pobl sydd â salwch meddwl difrifol [IPS-SMI] a Lleoliad â Chymorth Unigol Dibyniaeth ar Alcohol a Chyffuriau [IPS-AD], bydd y rhain yn parhau’n ddau wasanaeth Lleoliad â Chymorth Unigol ar wahân.

Fe fydd gwasanaeth Cysylltu â Gwaith yn cael ei ddarparu’n deg ar draws Sir y Fflint gyfan er mwyn i bob preswylydd gael mynediad/cyfle cyfartal i’r un lefel a safon o wasanaeth, waeth ble maent.

Suppliers are being sought to deliver Supported Employment Individual Placement and Support (IPS) contracts within the employability sector as part of the Connect to Work North Wales Programme, specifically in the County of Flintshire. The contracts will involve supporting people with disabilities who want to work to find local sustainable employment.

IPS integrates employment support alongside primary and secondary health services, and other support services. This integration ideally occurs at a physical level, through co-location and other policies, where applicable. IPS has been successfully utilised by areas working with DWP to deliver the ‘Individual Placement and Support in Primary Care [IPSPC] programme to support individuals with mild to moderate health and/or physical health conditions. CtW will utilise and expand on the approach taken with IPSPC. Whilst CtW will complement the existing IPS provision for people with severe mental illness [IPS-SMI] and IPS Alcohol and Drug Dependence [IPS-AD], these will remain separate IPS services.

The CtW service is to be delivered equitably across the whole of Flintshire so that every resident has equal access/opportunity to the same level and quality of service regardless of place.
