To commission a service that will support the discharge of admitted patients at up to 1 of the 3 sites within South Yorkshire and their associated sites, to their home address or stated discharge address if staying with family or carer, including patients whose permanent residence is at a residential or nursing home.
The reason for this VEAT notice is to extend the current agreement for approximately 1 year from the end of the current contract which expires on 31.3.2016. This is to give the CCG and potentially other CCG's an opportunity to complete a strategic review of patient transport services across South Yorkshire and Bassetlaw.
