The project is commissioned by Bridgend County Borough Council, Adult Social Care Services which is part of the Social Services and Wellbeing Directorate.

This is a brief to describe the requirements of the service and the expectations on the provider to:

- Review current social work structures, and pathways within adult social care

- Propose a new structure and implementation plan which would be evidence-based best practice and an analysis of current systems.

- Contribute to leading the agreed implementation plan to deliver a new professional social work model, streamlining and realigning associated pathways in adult social care.
