Abstract

The purpose of this service is to support people diagnosed with young onset dementia, their families and carers. This will be through a person-centred, asset-based approach to maintain the best possible quality of life and support to remain independent in their own homes and communities for as long as possible. This will be consistent, flexible care and support as agreed in the individual’s personal support plan and in relation to assessed need. This is a citywide service but will encourage people to maintain links within their local communities. The service will provide:

Day opportunities in a group setting – combination of centre based and community activities

One to one day opportunities (Community and Home Support Service) that facilitates engagement in activities of person’s choice – this should be provided locally to the individual according to the individual’s preference

Support Service and Carer and peer support group opportunities

Volunteer Befriending Service for all ages - this should be provided locally to the individual according to the individual’s preference

These services should be provided face to face unless the service user requests otherwise or as COVID or other restrictions dictate.

VI.3) Additional information

It is a mandatory requirement of this Contract that a prompt payment clause is present in all Contracts used in the delivery of the requirements. This ensures payment of sub-contractors at all stages of the supply chain within 30 days and includes an obligation to provide a point of contact for sub-contractors to refer to in the case of payment difficulties. This will require active monitoring of payment performance as well as the provision of evidence and reports to the Council on request or as contractually scheduled.
