Disgrifiad o’r Prosiect:
I ddarparu gwasanaeth gwaith wedi’i gefnogi sy’n cynnig profiadau dylunio graffeg ac sy’n seiliedig ar brint i bobl yn Sir y Fflint sydd yn byw â diagnosis iechyd meddwl. Fe fydd y gwasanaeth yn darparu cyfleoedd wedi’u personoleiddio i gael profiad mewn gweithle go iawn, magu hyder ac annibyniaeth, a gwneud cynnydd tuag at addysg bellach, cymwysterau, llwybrau galwedigaethol, neu gyflogaeth â thâl.
Fe hoffai’r Cyngor wahodd Darparwyr sydd â phrofiad o ddarparu gwasanaethau cefnogi o safon i drafod y gwasanaeth ymhellach. Fe fydd partneriaid ar gyfer y contract yma’n dangos gweledigaeth glir ar gyfer y gwasanaethau, gyda ffocws ar ganlyniadau unigol gan ddefnyddio cynlluniau sy’n canolbwyntio ar yr unigolyn a modelau dilyniant. Bydd partneriaid yn rhannu’r weledigaeth o gynnal gwerthoedd gofal cymdeithasol, cynyddu integreiddio a rhyngweithio rhwng gwasanaethau a chymunedau lleol drwy ddefnyddio partneriaethau masnachol, ymrwymiadau cymdeithasol a chytundebau gwirfoddol.
Mae hwn yn gyfle cyffrous i ddarparwr sydd ag arbenigedd a llwyddiant blaenorol mewn darparu gwasanaethau tebyg, i gymryd perchnogaeth a thrawsnewid gwasanaeth gwerthfawr ac sydd wedi’i hen sefydlu, gyda staff medrus a phrofiadol. Bydd disgwyl i’r darparwr a gomisiynir barhau i ddarparu cymorth a datblygiad o ansawdd uchel i bobl, gan gynnig gwerth am arian a chyflawni canlyniadau i ddiwallu anghenion y boblogaeth leol. Bydd angen i’r darparwr ddarparu cydbwysedd o gynnig cyfleoedd gwaith go iawn a chefnogi pobl sydd â phroblemau Iechyd Meddwl.
Mae’r gwasanaeth yn darparu gweithgareddau ystyrlon i bobl sydd ag anghenion cymorth cymhleth, gyda chyfleoedd i bobl gael gafael ar hyfforddiant a datblygu, i adeiladu sgiliau cymdeithasol a galwedigaethol.
Mae’n rhaid i’r Awdurdod weithio yn unol ag egwyddorion Deddf Gwasanaethau Cymdeithasol a Llesiant (Cymru) i hyrwyddo enghreifftiau cymdeithasol o ofal a chefnogaeth sy’n seiliedig ar werthoedd a bydd disgwyl i’r gwasanaeth barhau i weithredu fel menter gymdeithasol. Mae hyn yn cynnwys creu cyfleoedd ar gyfer cyflogaeth â thâl i bobl sy’n defnyddio’r gwasanaeth, cefnogi unigolion i ddatblygu eu potensial, a hyrwyddo annibyniaeth, dewis a rheolaeth.
Project Description:
To deliver a supported work service that offers meaningful graphic design and print-based experiences for people in Flintshire living with a mental health diagnosis. The service will provide personalised opportunities to gain real-world workplace experience, build confidence and independence, and progress towards further education, qualifications, vocational pathways, or paid employment.
The Council would like to invite Providers with experience of delivery in quality support services to further discuss the service. Partners for this contract will demonstrate a clear vision for the services, with a focus on individual outcomes using person centred plans and progression models. Partners would share the vision to maintain social care values, increase integration and interaction between services and local communities by utilising commercial partnerships, social commitments and volunteer participatory agreements.
This is an exciting opportunity for a provider with expertise and a proven track record in providing similar services, to take ownership and transform a well-established and valued service, with skilled and experienced staff. The commissioned provider will continue to deliver high quality support and development for people, offering value for money and delivering outcomes to meet the needs of the local population. The provider would need to provide a balance of offering real work opportunities and supporting people with Mental Health problems.
The service provides meaningful activities for people with complex support needs, with opportunities for people to access training and development, to build social and vocational skills.
The authority must work in line with the principles of the Social Services and Well-being (Wales) Act to promote social value- based models of care and support and would expect the service to continue operating as a social enterprise. This includes creating opportunities for paid employment for people who use the service, supporting individuals to develop their potential, and promoting independence, choice and control.
