II.2.4) Description of the procurement
Spinal cord stimulation is reserved for patients who have severe chronic pain that is limiting their daily life and who have already tried all reasonable, more conservative therapies, such as painkillers and physiotherapy. Spinal cord stimulation requires a minor surgical procedure under x-ray control in which a lead (medical wire) is precisely sited within the epidural space of the spine – for percutaneous wire insertion. If a surgical paddle electrode is inserted then a small window is created in the bone and ligament of the spine to place the electrode. In both cases, the lead is then connected under the skin to a battery device which is surgically sited in the buttock or abdominal wall. The battery powers the electrical stimulation of the lead in the spine. Once the system is set-up, it is controlled by the patient using a handheld controller. Many factors, including site and type of pain, are considered in determining which type of system to use.

• Anchors

Peripheral Nerve Stimulation (PNS) is a surgical approach to treat chronic pain in a small area of your body for patients with Neuropathy in the peripheral nervous system. Peripheral Nerve Stimulation involves surgery that places a lead (a wire-like electrode) next to peripheral nerves to control pain. The lead delivers rapid electrical pulses that are felt as a mild/pleasant tingling sensation called paraesthesia. The goal of this process is to reduce pain and reduce your medication requirements.

Instead of natural activation of ever changing firing axons of phrenic nerves in a randomised fashion, four compartments of the phrenic nerve is stimulated sequentially by means of four pole electrode during each inspiration. The four pole electrode(s) is surgically implanted around the phrenic nerve(s), leading to implant stimulator usually implanted in the chest wall. An external stimulus controller sends radio frequency signals to the implant stimulator by energy transfer coil which is worn over the implant stimulator.

II.2.4) Description of the procurement
Dorsal Ramus Medial Branch nerve stimulation is reserved for patients who have debilitating chronic low back pain, no indications for surgery and who have already tried all reasonable, more conservative therapies, such as back pain medication and physiotherapy. The stimulation target is the medial branch of the L2 dorsal ramus nerve which is situated outside of the spinal canal. The medial branch is a “mixed” nerve. It transmits pain signals from the spine joints to the brain, but at the same time it controls the spine stabilising muscles in the lower back. The medial branch is accessed via a percutaneous needle/introducer through which the leads are placed adjacent to the left and right medical branch. The leads are then connected to a battery powered stimulator which is surgically inserted under the skin of the buttock or lower back. Once the system is set-up, it can be controlled by the patient using a handheld remote controller.
Products used within a procedure may include but not limited to:
• Electrodes
• Implantable pulse generator
• Patient programmer
• Tunnelling tool
• Patient magnet
