The Provider will work in partnership with other members of the Multi-Disciplinary Team to provide:
• Specialist care for persons with long term clinical conditions;
• Specialist care for older people;
• Multidisciplinary/multi-agency assessment and care management planning;
• Return to home and follow-up care arrangements;
• Active, goal-based rehabilitation, enabling people as far as possible to return to their previous level of function and independence;
• Intermediate/integrated care;
• Post-operative rehabilitation and recovery;
• Medical assessment and treatment;
• Continued specialist therapy;
• Investigations to aid diagnosis; and
• Palliative and terminal Care.
