Cardiff Council is seeking to understand the market from providers who are committed to promoting services that meet the needs of, and promote and maintain the independence and inclusion of, older people and vulnerable adults who may have dementia, other forms of cognitive impairment, physical and sensory impairment and or mobility issues, whose needs will range from targeted individual support to complex needs.
Extracare and Supported Living recognises that all older people and vulnerable adults, regardless of the level or type of care need, are able to make choices about how to live their lives.
These services enable individuals to live as independently as possible. Care and support is personalised and outcome focused. The vision is for a locality approach based on the benefits of care services working closely with preventative services, community health and social work teams, community hubs and primary care clusters to achieve the best preventative and care outcomes for people.
Extracare and Supported Living takes into consideration specialist requirements for people who may be presenting with other care and support needs such as dementia, other forms of cognitive impairment and physical/sensory/mental impairment.
The extent of the services provided are dependent on the varying needs of the individuals and their outcomes, which are identified through their initial and ongoing care and support wellbeing assessments. The services provision is based on services delivered to meet different levels of outcomes.
Individuals who receive the services are:
• Older people generally aged 55 and over
• Older and Younger adults with a physical/sensory/mental impairment
• People with Dementia or other forms of cognitive impairment
The service is to provide individuals with the opportunity to live their lives independently, better manage and/or improve their physical and mental health, improve their overall quality of life, self-esteem and increase their independence.
Extracare and Supported Living, in this instance, describes a range of housing and support options which can be defined as:
• When an older person or vulnerable adult rents somewhere to live and receives Personal Domiciliary Care and Support from a domiciliary support provider based on site that is not the owner/landlord of the accommodation.
Extracare and Supported Living is generally where a group of flats are situated in the same block or proximity. People living in these properties can receive care and support hours targeted to their specific needs, whilst also benefitting from the background support on site.
The Care and Support Service provider will provide some housing management functions for some of the schemes and will enter into formal agreements with the landlords, setting out roles and responsibilities of each.
Below are details of the extracare and supported living schemes in Cardiff included in these services. The Providers develop a presence in the local community, building up links with community groups, networks and individuals to access community benefits and opportunities for the people who use the service.
Extracare Schemes
Llys Enfys
A total of 102 self-contained flats. Mix of 1 bedroom and 2 bedrooms
Of which 68 flats for extracare which include:
7 flats for older people with memory loss or dementia
8 flats for disabled young adults
2 flats for Respite/Reablement service (rental and associated costs by the Council)
34 flats are considered to be sheltered for older people and the services are not required to be delivered to those tenants residing in these 34 flats.
Plas Bryn
40 self-contained flats. Mix of 1 bedroom (27) and 2 bedroom (13)
All extracare older people.
Llys Y Goetre
60 self-contained flats. Mix of 1 bedroom and 2 bedroom including 10 flats used for Step -Up/Down service.
Supported Living Schemes
Dol-Yr-Hafren
15 self-contained flats 14 x 1bedroom and 1 x 2bedroom used as a respite flat. Older people/people with physical/sensory impairment/degenerative health conditions
Cwrt Hir
13 self-contained 1 bedroom flats and 3 self-contained bungalows.
Older people and people with physical disability, sensory impairment
Core Service Requirements
The Council expects the service to meet the range of care and support needs of all individuals, i.e. for those whose care and support needs are complex, such as those with dementia, other forms of cognitive impairment and physical/sensory/mental impairment and for those whose needs are less extensive though equally significant.
On-site care and support teams at the schemes will provide the service.
A minimum amount of personal domiciliary care hours will be required each week in each of the scheme. Domiciliary care will be delivered 24 hours a day, 365 days a year. The hours required will vary with the needs of individuals, identified through assessment and the Council’s eligibility criteria.
Day Hours - Care and support will be delivered between the hours of 7.00am to 11.00pm and are concerned with assisting the individual with personal care needs, together with attention to their general comfort and wellbeing.
Waking Night - It is anticipated that there will be a need for a wake-in provision which will be between the hours of 11.00pm to 7.00am, 365 days a year. This requirement will be subject to change depending on the prevailing needs of individuals.
Sleep In – If there were to be an identified need, the service may require a sleep in member of staff at a scheme to be available, as and when required. The sleep in applies for the hours 11.00pm to 7.00am 365 days a year.
At Llys Y Goetre, the purpose of the step-down flats is to build a bridge between hospital and home by supporting people to recover and resume independent living. The use of the step-down flats will be confined to individuals who no longer require acute care in hospital but would benefit from a period of ongoing targeted, social care intervention. This will also support a reduction in delayed transfers of care from hospital. Additionally, the flats can be used as intervention to prevent people being admitted to hospital and respite.
The Provider will be required to have capacity and flexibility to allow for emergencies that arise for the Council, individuals, and care staff, to ensure that calls are delivered as required in the care and support plan. The Provider will be required to respond to the immediate care and support needs of those returning to their homes following a hospital stay.
Care and Support Services are required to be delivered in accordance with the regulatory requirements of the Regulation and Inspection of Social Care (Wales) Act 2016 (‘RISCA’) and its regulations including The Regulated Services (Service Providers and Responsible Individuals) (Wales) (Amendment) Regulations 2019.
