The North Kent Clinical Commissioning Groups (CCGs) which comprise NHS Medway Clinical Commissioning Group, NHS Dartford, Gravesham and Swanley Clinical Commissioning Group and NHS Swale Clinical Commissioning Group are seeking to commission a community Ophthalmology service for patients registered with a North Kent General Practitioner (GP).

The aim is to bring care closer to home and ensure patients are seen by the appropriate healthcare professionals by improving access and increasing value for money. The service will enable patients to be diagnosed, treated and monitored within a local community setting, whilst addressing gaps and inequalities in service provision across North Kent.

The Contractor would be expected to work with the local health economy (primary and secondary care) to ensure a seamless transition between care providers, where this is required.

The North Kent CCGs approach is to commission a single lead contractor to offer community based ophthalmology service which offers triage, assessment, treatment and management of ophthalmic conditions in line with the inclusion and exclusion criteria outlined in the service specifications.
