Abstract

1.7 Enhanced care is described as the provision of care for patients who have been assessed by a registered practitioner and are at imminent and or significant risk of falls, unpredictable behaviours posing risk to themselves and or others, confusion, delirium, disorientation, agitation and absconding.

1.8 Provision of this care to work with and alongside multidisciplinary teams is required to ensure the specific care needs of vulnerable patients are met and delivered to a high standard, whilst maintaining safety, dignity and patient centred care.

The supplier is required to provide a reactive, efficient and professional response to any clinical area within the Trust that requires it.

The service will be able to demonstrate value for money through the delivery of affective care and support to patients. This will be monitored and evaluated throughout the duration of the service provision with KPI’s and reviews as stipulated.
