West Yorkshire ICB is undertaking a Market Engagement Exercise to assess the level of interest from suitably qualified providers to deliver a health inclusion service to vulnerable groups who reside within the Wakefield District.<br/><br/>The purpose of this service would be to engage with vulnerable groups to assess and address any immediate health and well-being needs and to support them to engage with mainstream health and care services.<br/><br/>This would be inclusive of services to refugees and asylum seekers, the homeless, sex workers and gypsy, Roma and traveller communities.<br/><br/>Current Delivery Model<br/>The health inclusion service would bring together numerous different aspects of current provision within a single model of delivery.<br/><br/>Anticipated Service Commencement Date<br/>01/07/2026<br/><br/>The high-level objectives of the service are:<br/><br/>Improved Access to Healthcare<br/>• To increase the extent to which socially disadvantaged populations engage with health and care services.<br/>• Reduction in barriers to primary care, screening and prevention services<br/>• Increased registration with GP’s and uptake of routine health checks<br/>• Meet unmet needs within the population <br/>• Signpost and refer to wider services.<br/><br/>Better Health Outcomes<br/>• To improve the self-reported health outcomes from the socially disadvantaged populations.<br/>• Support the system to lower mortality rates and improved management of long-term conditions<br/>• Enhance mental health and physical health support<br/>• Reduction in emergency service use through earlier interventions<br/>• Support service users to receive care at the right place at the right time.<br/><br/>Enhanced Patient Experience<br/>• Support the system to provide more culturally competent and trauma-informed care<br/>• Support the reduction of stigma and discrimination in healthcare settings.<br/>• Reduce barriers to service users accessing mainstream healthcare services<br/>• Empower the patient voice to improve quality of care within services<br/><br/>System Level Impact<br/>• Reduce attendances to urgent and emergency services<br/>• Support effective discharge planning <br/>• Support the local authority on their statutory responsibilities within public health and housing<br/>• Work within the system to ensure streamline pathways and processes are in place for effective care planning.<br/>• Support service users to use NHS resources efficiently through targeted, proactive care<br/>• Support the development of neighbourhood health and MDT’s within the health inclusion populations.<br/>• Be a leader for health inclusion in the system and support services in the delivery of healthcare for these populations.<br/><br/>Workforce Development<br/>• Provide training in trauma-informed approaches, cultural competence and health literacy.<br/>• Workforce development supports effective communication between health, social care and voluntary sector partners.<br/>• Increased knowledge of barriers faced by health inclusion populations across the system.<br/>• Staff should complete the appropriate level of the WYICB health and social care inclusion health workforce training <br/><br/>Community Engagement and Co-Design<br/>• Support continual service development working with health inclusion groups<br/>• Facilitate strong partnerships with voluntary, community and social enterprise (VCSE) sectors<br/>• Ensure the patient voice is represented in service development<br/>• Ensure peer advocacy and lived experience are part of the service.<br/>• Improve levels of participation from health inclusion populations in feedback and evaluation.
