Homecare is considered any care and support a person might require in their own home, including preparing meals, taking their medication, and attending their personal care needs.

The Service Provider will adhere to the following Service aims and requirements throughout delivery of the Services for the following;

• Older adults

• Adults of all ages with organic mental health problems including dementia (includes those who exhibit behaviours that challenge)

• People who may be physically frail or with physical disabilities

• People who have sensory loss

• People who are entering the end stages of life

• People with learning disabilities

• People with autism or other diagnosed neurodiversity

• People with cognitive impairment

• Discharge & Transition Service

• People who are entitled to Continuing Health Care funding

• People in need of specialist health tasks In addition to the above, support may also provided to family carers with any of the above disability, subject to to a carers assessment as part of the Care Act 2014.

The service provider is to support people who are living at home, and to focus on achieving their agreed outcomes. These People may have been through a period of reablement with another care provider, and strong links will need to be established between Providers to ensure the smooth transition of care/support for People where appropriate, ensuring People continue to be empowered and enabled.

This includes day time care, night time care, waking nights, sleeping nights and live-in care.

In these cases, standard interventions are insufficient and require specific expertise and a bespoke, multidisciplinary approach to care.

Extra Care Housing

II.2.4) Description of the procurement

Extra Care Housing (EHC) are specialist housing schemes for older people that have been specifically designed to maximise independence. All tenants have their own apartment with a front door and yet also benefit from the availability of the 24/7 onsite care and support service.

Extra Care Housing provides an integrated housing, care and support solution whereby people can benefit from having care and support services available to them within the housing scheme they live in. Service Users will be enabled to: remain in their own homes, with a service that meets their varying and increasing care needs; access appropriate community facilities; at all times remain as independent and self-caring as possible.

Extra Care Housing should provide a community where people aged 55 and over with different levels of capabilities can be supported. Not all Service Users will have a care need, but they may benefit from living in a more supportive environment. All Service Users benefit from the additional security of having a Waking Night Support service. Some Service Users will be Service Users of the Council and will have formally assessed care and support needs. Tenancies will be allocated in accordance with this ethos of promoting a 'balanced community' within Extra Care Housing.

Where a person has eligible care needs and requires support in addition to standard or complex care at home, services can be called off from this Lot. This support can include, but is not limited to, the below:

Individual support:

• Where a Person may need rehabilitative support for a short period of time. This might include following a stay in hospital, recovering from a stroke or acquired brain injury. This might include supporting them in relearning day-to-day living skills and working together for the person to become more independent.

• Where a Person may need support in the community to go shopping, attend groups or activities, and achieve more holistic outcomes.

• Where a Person may need support at home that is not considered 'typical' homecare. The Person might be able to meet their own personal care needs, but have additional needs outside of the home or living independently.

Floating support:

• This might include a support worker who delivers day-to-day support to a group of People living in an area.

• It may also include someone who needs to be specially trained and have an understanding of a specific People's (ie client group's) needs.

• It may be location based or building based, but can include a number of different People being supported to achieve specific outcomes.

Open procedure

IV.1.11) Main features of the award procedure:

The procedure followed was similar to the open procedure, but relied upon the flexibilities provided by the Light Touch Regime to deviate from the full scope of provisions applicable to the Open Procedure as described Regulation 27 of the Public Contract Regulations 2015.

Tenders were evaluated 100% on quality, with pricing only being provided for information purposes (and to then be used during the call-off process once the PDPS is established).
